For treatments other than iron, requests go through the PSP. If you already have a case manager, ask them to contact the team to arrange your treatment.

Have the exact medication and program names, case manager contact details and prescriber information ready. The team will explain how to provide the necessary documents.
If you do not have a program yet
Contact the clinic with your prescription to ask about the next step. Your prescriber or pharmacy can also help identify the program associated with your medication.
Confirm arrangements
Before the appointment, clarify who supplies the medication, which arrangements are complete and whether any costs remain payable by you. Arrangements may vary by product and program.
